During your government registration to electronically prescribe, we are required to verify your professional identity. This is to protect the safety of all providers’ identities and information. As part of this process, we use the Equifax eIDverifier that allows us to ask questions that only you would know the answers to. These include questions regarding past addresses or financial history. We do NOT run or check your credit, and your credit will not be impacted by this appointment.
However, if you have placed a credit freeze with the three major credit bureaus, it also blocks the Equifax eIDverifier tool. We cannot proceed with verifying your professional identity as mandated until you contact Equifax to lift the freeze. Please temporarily lift any credit freeze at least one hour prior to your confirmed appointment time.

What to Expect
The Identity Verification session is extremely brief, usually about a 5-10 minute phone call with us. After we complete your verification, you will be able to reinstate your freeze with Equifax.
