Record a Diagnosis or ICD10 code
1. Log in to your iCoreRx website
How to Change Your Prescribing Location on Semi-Integrated Sites (Standalone)
2. Select a Patient.
3. Select the Problems tab in the top menu bar.

4. Click +Add button in the Diagnosis section.

5. Enter the diagnosis name in the Search Diagnosis field, then click the Search button.

6. Click the + icon to select the diagnosis.
a. Click on the Diagnosis Code link to open Medline Plus documentation for the diagnosis.

7. The Save Diagnosis window opens, confirm details and click the Save Diagnosis button.

8. The diagnosis is displayed in the patient’s Diagnoses list.

a. The Diagnosis column lists the diagnosis name.
b. The Code column lists the ICD code of the diagnosis.
c. The Std column indicates the type of diagnosis code.
- Currently, only ICD-10 is supported.
d. The Physician column lists the name of the doctor who recorded the diagnosis or the doctor that the user was working under.
e. The Onset column indicates the onset of the diagnosis.
f. The Recorded column indicates the date the diagnosis was recorded in the system.
Associate Diagnosis (ICD10 code) to Patient’s Prescription
1. On the Prescription Pad, at the bottom.
2. Click the dropdown under Associated Diagnosis.

3. All diagnosis’s that are recorded from the Problems tab will be listed.
- Select the appropriate diagnosis for the patient’s prescription.
4. You can do the same for Secondary Diagnosis.
Edit or Delete a Diagnosis
1. Click the Edit button for the desired diagnosis.

2. Make desired edits, then click the Save Diagnosis button.

3. To Delete a diagnosis, click the down arrow and select Delete.
- Deleted diagnosis displays in the Diagnosis History section.
